I have been playing on and off since the original release. I seem to remember Tokens of the Twelve dropping from all quests that were level 20+ even if it was just from the Difficulty of the quest. Now they only drop from 26 quests and the greater tokens from only 3 raids. ALL other Reincarnation items are earned with commendations of valor.They give the same tokens on ER1 as on EE afaik and those drop rates seem about what they've always been, confirmed by this table. They seem to be all same or worse than LoD and that one gives fragments a lot of the time too, so there might be a chance at a full token but usually just fragments.
